MyBones will email you a declaration form. The declaration must be completed and returned to MyBones before the sale can commence.
In the UK, if you have been diagnosed with any of the following conditions you may claim VAT relief on the Marodyne LiV device:
If you’re not sure whether your condition means you’re chronically sick or disabled, you may wish to consult a doctor or other healthcare professional.
To claim VAT relief, you must complete the HMRC corresponding declaration and email/send it back to MyBones, confirming you have been diagnosed with a chronic medical condition that qualifies for VAT relief and confirms you have read and understood these T&Cs.
The person making the claim should complete this form, but a family member, carer or medical professional may complete the declaration on the claimant’s behalf.
By confirming that you are eligible for VAT relief, you acknowledge that it is your responsibility should HMRC query your declaration and that VAT officers may contact you for evidence.
MyBones accepts no responsibility or liability for false VAT exemption claims.
Please note VAT relief is only available for purchases made for personal or domestic use. Purchases intended for business/clinical use are not eligible. Please see the HMRC guidance for more information. VAT relief for chronic illnesses and disability is only available for UK residents. All other VAT calculations will be in accordance with local VAT regulations.
